My brother and my dad are so good with realism illustrations, I'm so proud, and totally jealous :) Give them a piece of paper and a pencil, and(with enough time) they can get photo quality. I never quite got there in my own graphic art program; my stuff always maintained a stylized quality I couldn't get rid of.
The skill comes with pencil and paper, then you learn how to do it in the programs. I can run laps in Photoshop/Illustrator/In Design(Adobe Creative Suite), but it's all going to come out fanciful and stylized(not realistic). I never quite got my eye developed to the same extent as the menfolk :) But that's okay! I love seeing them do it!
